This ship was launched in 1964 as USNS Kellar:

During the hurricane Betsy in September 1965, still unfinished and unmanned, she was hit by a drifting crane barge and one or more merchant ships, got torn from moorings, rolled over and sank. A month later Kellar was finally refloated and town to a wharf for a reconstruction due to severe damage. Some of the major changes were extension of the superstructure, removal of the bow thruster and addition of an active powered rudder.

In 1972 she was transferred to Portugal under the Security Assistance Program as NRP Almeida Carvalho. Between then and the 1990's she conducted the most research surveys for both Navy and scientific community.

I can add that it seems the Almeida Carvalho is being prepared to be sunk in the Algarve to create an artificial reef. Several decommisioned vessels of the PN are going through this procedure. Last October the corvette Oliveira e Carmo and one Cacine patrol ship were sunk in front of Portimão:

These patrol boats were designed for Portuguese Timor but they ended up in Africa serving during the Colonial Wars. Eventually these small ships were either scuttled or transferred to the newly formed Republic of Angola.
