What left of a steam turbine after they connected the generator to the grid without ensuring that it was in synchronism with it.
As a result the generator tried to turn 180 degrees instantaneously. They found a piece of the shaft in the next room.

On facilities this size they don't rely on operators manuals, they send the operators to school.
