A few problems:
1. Blast from the Y mount.
2. I just see a Terrier mount in place of the X turret. Where is your magazine, reload handling. See the Cleveland CG conversions, Belknap and Leahy class.
3. I think you may need 2 target illuminators.

If I recall correctly the 5.25" had a somewhat disappointing rate of fire.

The Buccaneer also operated off the HMS Hermes, a modified Centaur class light carrier. That was a plane about the size of an A-6 Intruder.

As for the "odd caliber" for a light cruiser, OTO built a 135mm/45 DP for the Guisseppe Garibaldi cruiser as part of the conversion to a CG.

The Hustler was a one trick pony - nuclear strike only, with the payload in the weapons/fuel tank pod. theoretically an SM-2 Block III may be able to fit in a Mk-13 launcher - within a few inches to spare.
